What is SH Mobile?

Supporting Continuity of Care Beyond the Hospital

​SH Mobile Veterinary Nursing Services provides in-home veterinary nursing support for patients who may benefit from ongoing care but experience difficulty attending traditional veterinary appointments due to fear, anxiety, mobility limitations, chronic disease, or complex home-care needs.

The goal of SH Mobile is to help patients remain connected to veterinary care while supporting the treatment plan established by the attending veterinarian.

Identify a Patient Who May Benefit

The attending veterinarian identifies a patient whose medical needs, mobility limitations, stress levels, or home-care requirements may make in-home nursing support beneficial.

Referral and Consultation

The client is provided with SH Mobile's contact information and schedules a consultation to discuss their pet's needs, goals, and suitability for home-based care.

Ongoing Collaborative Care

Services are provided in the patient's home while supporting the treatment plan established by the attending veterinarian. Visit summaries and observations can be communicated back to the veterinary team as appropriate.
